Les joueurs dplacent tour de rle leurs sectateurs pour activer divers sites sur les plateaux,

Evil High Priest est un jeu de placement de travailleurs dans lequel les joueurs jouent le rôle de prêtres d'une entité Lovecraftienne. Tous les joueurs adorent le même Grand Ancien, mais ils ne sont absolument pas des alliés. Quand ils réveilleront leur Grand Ancien, il décidera lequel d'entre eux est le meilleur (basé sur les PV) et le nommera le Grand Prêtre.

Les joueurs déplacent à tour de rôle leurs sectateurs pour activer divers sites sur les plateaux, rassemblant des ressources, invoquant des monstres, brisant des signes anciens et lançant des raids d'investigateurs (qui attaquent tous les joueurs). Lorsque tous les joueurs sont à court de sectateurs, la phase d'action se termine et les joueurs récupèrent leurs cultistes, libérant les sites du plateau pour la prochaine phase d'action. La victoire est déterminée lorsque le dernier signe aîné bloquant votre grand vieux est brisé, et à ce stade, tout le monde marque des points.
